About The Position

People Incorporated Mental Health Care Services is seeking a Case Manager II to join our Children’s Targeted Case Management team. This is a full-time hybrid position, helping individuals under 18 years of age gain access to medical, social, educational, vocational and other necessary services connected to the person’s mental health needs. Eligible children must have a mental health diagnosis. This is a full-time hybrid position located in the Twin Cities, Minnesota. Who We Serve Children age 5-17 years of age Children served are struggling with functioning at home, in school or in the community due to a serious mental health diagnosis Our Services: Assessing the child’s needs, goals and the impact of mental illness on their everyday life Referring and linking to resources, supports and services, including: individual and family therapists, primary care physicians, individual and/or family Children’s Therapeutic Services and Supports (CTSS) skills workers, individualized education plan (IEP) evaluation, health insurance and psychiatrists Monitoring the effectiveness of the resources, supports and services being utilized Discussing the progress made toward goals and recovery Hours: Full-Time, Monday-Friday typical business hours, some late afternoon and early evenings as needed Location: Hennepin, Anoka and Dakota Counties | Hybrid with remote opportunities Hiring Range: $24.25 - $25.75/ Hourly, depending on qualifications Hiring Bonus: $1,000 available to external candidates People Incorporated Mental Health Services is the largest nonprofit mental health provider in Minnesota. Requirements

  • Passing all background and requirements for facility and position; and
  • The ability to pass the screening required by Federal Human Services Office of Inspector General to assure that person has not been excluded from working in the Health Care Programs which receive federal funding.
  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in behavioral science or related field AND One Year (2,000 hours) of full-time providing mental health services to children and their families under supervision of a licensed mental health professional OR Master's degree in behavioral science or related field

  • Knowledge of adolescents mental and chemical health issues, and awareness of community-based mental and chemical health resources
  • Experience with children/ adolescents and families
  • Ability to forge a mutually respectful partnership with persons served and their families
  • Knowledge of local community resources and how to use those resources for the benefit of children and their families

Responsibilities

  • Act as an internal resource to other Case Managers providing on-site counsel and establishing best practices on program expectations.
  • Develop and maintain subject matter expertise in a particular area that allows for continued client care and effective sharing of information to others so they may implement.
  • Provide temporary case management services during program transitions to ensure that program service levels are maintained.
  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Maintain regular and active contact with clients ensuring services sought are utilized and partnering with the client to determine effectiveness of the plan and next steps.
  • Provide general knowledge and guidance on assisting clients in the multiple phases of applying for and receiving services and benefits.
