About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Requirements

• High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or
• Associates degree and one (1) year of experience; or
• Bachelor's degree in Social Services.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups.

Responsibilities

• Provide case management services to individuals with mental illness and co-occurring disorders.
• Assist clients in living independently in their chosen community.
• Conduct wellness checks and facilitate wellness groups.
• Engage in skill-building activities and housing stabilization focused case management services.
• Provide behavioral health and substance use services.
• Conduct crisis interventions and conflict resolution.
• Mediate with property managers to support clients in maintaining housing.
