About The Position

Under direct supervision, the Case Manager II serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community.

Requirements

  • High School Graduation or G.E.D. equivalent and two (2) years of direct services in Mental Health or community services; or
  • Associates degree and one (1) year of experience; or
  • Bachelor's degree in Social Services.
  • Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
  • Willingness to use personal vehicle for client transportation.

Responsibilities

  • Provide case management support to individuals with mental illness and co-occurring disorders.
  • Assist clients in living independently in their chosen community.
  • Drive clients to appointments and group sessions using personal vehicle.
