Requirements

  • Bachelor’s degree from an accredited university with a major in psychology, sociology, social work, counseling, or other related human services field for QMHP credentialing
  • For Case Manager II, minimum one (1) year of experience in behavioral health case management
  • For Case Manager III, minimum two (2) years of experience in behavioral health case management
  • Graduate work in a related field may be substituted for up to one (1) year of experience
  • Must have a valid Texas driver’s license and be insurable under agency vehicle policy
  • Must be 18 years of age or older

Responsibilities

  • Maintain ethical and professional communications at all times using respectful Trauma-Informed Care language
  • Conduct crisis response and coordination with youth and families experiencing or who are at-risk of experiencing a crisis episode
  • Conduct comprehensive life needs assessments (CANS/NOMS) taking into account needs as viewed by the child/family members, & school personnel in a timely manner
  • Provide any community referrals needed, and follow up with the family, referral, and community resource to ensure need was met
  • Provide skills training using evidenced based curriculums
  • Advocate on behalf of the child/adolescent served so that access for required services are available and that services rendered in a timely fashion and in accordance with the child's abilities
  • Provide outreach in community when needed to inform others about the Family & Youth Diversion Program
  • Spend 50% of time with families and youth/providing services to families and youth
  • Maintain same-day next-day documentation requirements and have a 95% or higher compliance average for all families served
  • Be willing to travel to provide services throughout the Texas Panhandle
  • Execute prudent action in emergency situations and help team members when families need crisis response or other diversion-based services
  • Adhere to TPC Business Code of Conduct and report concern if ethical and/or business standards defined in the code are compromised
