Requirements

Strong interest in working with youth in behavioral health or residential settings
Ability to coordinate services and manage multiple priorities
Strong communication, organization, and problem-solving skills
Commitment to trauma-informed, strengths-based, and client-centered care
Ability to work both independently and as part of a team
High School Diploma or equivalency certificate.
Valid driver’s license (Class C or D)

Responsibilities

Provide direct case management services to youth in residential care.
Support implementation of individualized service and treatment plans .
Connect youth and families to community resources and natural supports .
Coordinate care with internal teams and external providers (e.g., medical providers, schools, and state agencies).
Assist in planning for safe and stable discharge from residential care.
Coordinate and follow up on aftercare services to support continued success.
Serve as a liaison between families, treatment teams, and community partners .
Actively participate in multidisciplinary team meetings and case conferences .
Communicate updates and concerns in a timely and professional manner .
Work in the residential milieu as needed to support program operations and youth care .
Respond to crisis situations and coordinate appropriate interventions.
Monitor and address safety risks within the program environment.
Maintain training in CPI/de-escalation, First Aid/CPR, and medication protocols .
Maintain accurate and timely documentation in Electronic Health Records and required systems .
Ensure compliance with HIPAA, agency policies, and best practices .
Track services, progress, and outcomes to support quality care.
