About The Position
New River Valley Community Services is seeking a Case Manager I or II at New Horizons! The Case Manager provides case management for clients of the crisis stabilization program, including clients with mental illness and co-occurring substance abuse. The Case Manager coordinates services, resources needed by the client during residential services, and monitors outcomes up to 30 days post discharge. This role also provides coordination for discharge services with agency and community resources. In this role, you will coordinate with Special Justices, program psychiatric staff, and Independent Evaluators for commitment hearings held on-site.
