New River Valley Community Services Emergency Services is seeking a Case Manager! The Case Manager I or II for Emergency Services is a mid-intensive position that provides phone triage, screening services, referral, and support for individuals with Mental Health, Substance Use, Intellectual Disabilities, and Co-Occurring Disorders. This position refers individuals in crisis to Emergency Services staff, completes phone screening documentation to identify potential treatment needs and appropriateness for NRVCS providers, providing services to the clients as well as to their families/care-givers and may include telephone crisis intervention and/or empathy. The Case Manager would also be responsible for coordinating services and/or resources needed by the consumers to support their basic needs and enable them to remain and function optimally in the community.
