About The Position

If you answered YES , then SAY San Diego s Family Connection Hub program may be a great fit for you! A Care Navigator provides assessments, referrals, linkages, and follow-up support to strengthen and improve the well-being of children and families across San Diego County. Position is eligible for hybrid work schedule not to exceed 60% remote and includes working from two offices located in two regions of San Diego County.

Requirements

  • Experience providing case management/case work services to underserved or special needs populations with varied and complex health, behavioral health, economic and educational circumstances.
  • Demonstrated experience working with diverse populations and cultures and socio-economic families within collaborative environment
  • Experience compiling statistical data monthly.
  • Experience living or working in community to be served, including awareness of available resources, community conditions, barriers, and assets.
  • Experience working with child welfare, probation and behavioral health services populations.
  • Must have access to reliable transportation and be willing to use it for job-related tasks.

Responsibilities

  • Provide short term case management services through a client-centered approach utilizing Motivational Interviewing techniques to complete identified tools for needs assessment.
  • Manage full case load of parents/caregivers to include intake, client strength/needs assessment, referral, and follow-up services.
  • Maintain accurate client files, document case notes in timely manner, and complete funder reports as requested.
  • Schedule and conduct home visits, office/community visits, telehealth or telephonic sessions with clients for intake and or client servicing appointments based on client preference.
  • Develop and maintain positive relationships with clients and other connected program partners.
  • Receive referrals and contact families within 24 hours.
  • Attend monthly multidisciplinary meetings.
  • Complete relevant training including Motivational Interviewing, Mandated Reporter training, North Carolina Family Assessment Scale for General Services, Trauma/Trauma-Informed Services, Cultural Competency, Mental Health First Aid, and others as assigned.
  • Maintain positive relationships with colleagues, referral sources, funding sources and other collaborative partners.
  • Attend and participate in partner and funder-related meetings.
  • Participate in outreach activities including community-based events.
