About The Position

The Case Manager I – Adult Medicine works as part of a multidisciplinary health care team to provide comprehensive, patient‑centered services in a community health setting. This role supports adult primary care patients to promote continuity of care and improved health outcomes.

Requirements

  • High school diploma or General Educational Development (GED) certificate, and completion of a State-approved program for Medical Assistants
  • Completion of state-approved training and certification (Calif. Business & Professional Code 2069, 2070; and Calif. Code of Regulations 1377, 1377.1) relating to the performance of injections, skin testing, and withdrawal of blood
  • One (1) year minimum experience with Adult Medicine or Family Practice, counseling, health education, client advocacy, and community networking
  • Maintain a current Cardio-Pulmonary Resuscitation certification, in compliance with corporate CPR Policy
  • Demonstrated competency in understanding and addressing population‑specific needs when serving adult medicine patients
  • Maintain or exceed competency levels as established by corporate standards for clinical practice
  • Possesses excellent organizational skills and the ability to prioritize
  • Ability to work both as a team member and independently
  • Ability to perform a variety of tasks once trained
  • Willing and able to work a variable, flexible schedule, Monday through Saturday, and evenings
  • Fluency in English (speaks, read and write)
  • Fluency in Spanish (speaks, interpret, read and write)
  • Current California driver’s license, appropriate insurance coverage and a driving record acceptable to the NEVHC’s insurance carrier (if required to drive on the job)

Responsibilities

  • Provide assessments for adult primary care patients
  • Engage in care planning for adult primary care patients
  • Perform case management for adult primary care patients
  • Coordinate appointments for adult primary care patients
  • Follow-up on labs and referrals for adult primary care patients
  • Maintain documentation for adult primary care patients
  • Provide patient education
  • Offer counseling to patients
  • Conduct outreach to promote continuity of care and improved health outcomes
