Opportunity awaits! We are hiring an Case Manager (Human Services Case Manager) to join our team. In this position, you will assess Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Summary of Duties As an Case Manager, you will: Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact. Work closely and collaboratively with the Oregonian, other agencies, medical and care providers, and community partners to provide benefits, services, and information. Work with Oregonians to create person-centered risk mitigation plans and provide risk mitigation as needed. Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs. This can include collateral interviews both in person and on the phone. Develop detailed, individualized, and person-centered case planning for Oregonians. Inform Oregonians of rights, responsibilities, service options, and methods of recourse. Utilize all applicable forms, computer systems, tools, and any other available resources.

Requirements

  • A valid driver’s license and acceptable driving record are required for this position.
  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field OR A bachelor's degree in any field AND one year of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing) OR An associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing) OR Three years of human services related experience (i.e., work providing assistance to individuals and groups with issues such as economically disadvantaged, employment, abuse and neglect, substance abuse, aging, disabilities, prevention, health, cultural competencies, inadequate housing).
