About The Position
The Case Manager provides a range of comprehensive community-based case management services, including housing services and trauma-informed case management for individuals who are homeless and/or returning to the community from incarceration. The position serves as a liaison to the District of Columbia Department of Corrections, READY Center, Anchor Mental Health (Core Service Agency), and Housing, to identify and coordinate community resources and make the appropriate referrals. The Case Manager establishes and maintains effective working relationships among area residents, and property owners in the community.
