About The Position

Youth that are court-ordered to be on home incarceration or home supervision are processed by Department of Juvenile Justice/Detention Alternatives Coordinators (DAC). The DACs then make referrals to the Kentucky United Methodist Children’s Homes case managers. The youth will be monitored by the Case Manager to ensure the completion of the court mandates - this monitoring could include GPS monitoring equipment placed on the youth and in the home. The Case Manager will monitor the youth weekly through face-to-face contacts and as DAC specifies. Additional daily contacts will be in the form of telephone calls to specified locations depending on the individual situation. Reports regarding the youth’s performance on this mandate will be provided weekly to the referring DAC.

Requirements

  • Must be at least 21 years old and have a high school diploma.
  • Must have a valid Kentucky driver’s license and submit to an intensive background investigation.
  • Must have reliable transportation for extensive travel.

Nice To Haves

  • Previous case management or juvenile services experience preferred.

Responsibilities

  • Provide contact with the youth, as identified by the DAC including but not limited to one face to face contact per week with daily telephone contact.
  • Remain available via cell phone for on-call responsibilities at all times, or communicate with Director of ATD Services when not available.
  • Receive and process referral/intakes as required by funding source.
  • Assure proper knowledge of tracking equipment use and maintenance.
  • Accountable for the proper documentation and timely completion of all paper work associated with the requirements of the position, including the collection of date for: monthly billings, weekly Progress Reports, budget and year-end program reports.
  • Attends professional meetings, workshops, seminars, and other training sessions necessary to maintain professional expertise.
  • Communicates all pertinent information to the Director of ATD Services.
  • Maintain collaborative relationships with community workers and DAC for case planning.
  • Keep abreast of issues facing youth today.
