Position Details: Schedule: 32 hours per week with the hours typically 8:00 AM - 4:30 PM, Monday - Friday, with flexibility to create your own daily schedule based on patient load. Description : Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.

Requirements

  • Diploma from an accredited school/college of nursing.
  • Required professional licensure at time of hire.

Nice To Haves

  • We prefer one year of experience in Med/Surg, ER, or ICU to ensure our patients receive the best care possible.

Responsibilities

  • Implements and monitors patient care plans.
  • Monitors, records and communicates patient condition as appropriate.
  • Performs direct participant care, including acute triage, blood draws, wound care, etc.
  • Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care.
  • Serves as a primary coordinator of all disciplines for well-coordinated patient care.
  • Carries out physician and nursing orders.
  • Attends team meetings and care plan meetings.
  • Assesses and coordinates patients' discharge planning needs with members of the healthcare team.
  • Participates, develops, and provides education to participants, caregivers, and associates.
  • Participates in after hours/weekend on-call rotation, as needed.
  • May require travel to participant place of residence, long term care and assisted living facilities.
  • Other duties as assigned
