About The Position

The Case Manager for DHS Homeless Housing and Mental Health at Mental Health America of Los Angeles (MHALA) is responsible for providing high-quality Intensive Case Management Services (ICMS) to clients eligible for permanent housing. This role focuses on a client-centered approach, assisting individuals experiencing homelessness with medical and behavioral health challenges as they transition into permanent supportive housing. The position emphasizes collaboration, communication, and a commitment to the well-being of both clients and employees, fostering a supportive work environment.

Requirements

  • High school diploma or GED with a minimum of 1 year of experience providing psychosocial rehab services to a disabled population.
  • Bachelor's degree in a human service or related field preferred.
  • Current class C driver's license, a vehicle for transportation, and proof of valid automobile insurance.
  • Proficient in Microsoft Word, Excel, and Outlook.
  • Ability to learn and utilize Homeless Management Information Systems (HMIS) and Electronic Health Records Systems (EHR).
  • Experience working with clients with mental illness, chronic health issues, and substance use disorders.
  • Expertise in chronic homelessness, outreach and engagement strategies, intensive case management services, and crisis intervention.

Responsibilities

  • Assist individuals at every stage of the housing stabilization process with flexible services tailored to client needs.
  • Conduct outreach and engagement, processing referrals from DHS and other partners, and establishing rapport with clients.
  • Perform intake and enrollment activities with eligible clients, assisting with program eligibility documentation and project intake forms.
  • Provide temporary housing referrals until permanent housing is secured.
  • Ensure all documentation is prepared and entered into appropriate systems monthly.
  • Assist clients with move-in processes to permanent housing, including document readiness and coordination of move-ins.
  • Conduct comprehensive psychosocial assessments and develop individualized case management service plans in collaboration with clients.
  • Ensure clients have access to health, mental health, and substance use disorder services.
  • Assist clients in establishing a medical home and maintaining continuity of care.
  • Support clients in obtaining income, benefits, and securing employment or educational activities.
  • Provide housing location services and educate clients on tenant rights and responsibilities.
  • Offer eviction prevention counseling and work with property management to resolve housing stability issues.
  • Complete all paperwork in compliance with applicable laws and standards, documenting in EHR with a 24-hour turnaround.
  • Engage in professional development activities to enhance personal and professional skills.
