Case Manager for Developmental Disabilities (Hybrid Schedule) Join a dynamic Supported Living agency and work with a team of supportive, hardworking Case Managers. The Case Manager is responsible for the coordination of supported living services for adults with developmental disabilities, with a maximum caseload of 10 individuals. The Case Manager coordinates and assigns schedules for support staff, live-in staff, and IHSS staff. In coordination with the Director and Associate Director, the Case Manager will supervise, train, monitor, and evaluate all paid support staff. It is critical that the Case Manager understand, model, maintain, and articulate the philosophies, values, and goals of supported living as they relate to each individual and their lifestyle. The Case Manager will be part of a team that includes the individual receiving services, service professionals, family members, and others identified in the Individual Support Plan (ISP). Excellent written and verbal communication skills are a must.
