About The Position

The Case Manager role within Adult Outpatient Programs focuses on providing clinical interventions to adults in a community setting. The primary aim is to support recovery, symptom reduction, and stabilization while preventing the need for higher levels of care. Case Managers work directly with consumers in various environments, including homes and schools, to monitor their well-being and facilitate access to necessary resources.

Requirements

• Bachelor's degree in a mental health discipline.
• Completion of case management training certification by Alabama State Department of Mental Health and Mental Retardation post-hire.
• Valid state-issued driver's license and proof of current liability insurance.
• Knowledge of psychiatric disorders in the adult population within six months of employment.
• Knowledge of cultural diversity and behavior management techniques.
• Understanding of crisis intervention and non-violent crisis intervention practices.
• Familiarity with Psychiatric Rehabilitation principles and available community resources.
• Knowledge of educational methods suitable for adults with severe and persistent psychiatric disorders.
• Understanding of rules and regulations regarding benefit entitlements such as social security, Medicaid, and Medicare.

Responsibilities

• Assist with monitoring consumer's well-being in mental health, physical health, living, learning, working, and social environments.
• Travel to consumers' residences, including homes and community-based living facilities, to provide monitoring and support.
• Empower consumers and their families with information, education, and support for psychological adjustment.
• Provide appropriate linkage to community resources.
• Assess consumer's needs and monitor their progress.
• Secure placement for consumers in the least restrictive environment.
• Identify goals and plan for appropriate services.
• Advocate on behalf of the consumer to secure community resources.
• Evaluate consumers for discharge planning.
