About The Position
The Case Manager at CAMBA's Flagstone Family Center plays a crucial role in providing comprehensive case management services to families in a Tier II transitional residence program for homeless families. This position is essential in ensuring that clients receive the necessary support and resources to stabilize their living conditions and work towards permanent housing. The Case Manager will be responsible for reviewing all documentation that establishes clients' eligibility for the program, ensuring that all necessary files are accurately maintained and updated. In addition to administrative tasks, the Case Manager will conduct initial intakes and psycho-social assessments to identify the needs of clients and their families. This involves collaborating with clients to prepare and revise independent living plans that outline both short-term and long-term goals. The Case Manager will also be tasked with scheduling appointments for clients with various referral organizations, assisting them in completing applications for benefits and entitlements, and monitoring their progress towards achieving their goals through regular contact and documentation. This position requires a proactive approach to client management, as the Case Manager will need to maintain ongoing communication with clients, conduct home and office visits, and document progress through detailed notes. The role is vital in helping families navigate the complexities of the shelter system and access the resources they need to thrive. The Case Manager will report directly to the Assistant Program Manager and will work within a supportive team environment at the Flagstone Family Center, located in Brooklyn, New York. This full-time position offers a salary range of $41,000 to $45,000 annually, depending on experience and qualifications, and includes a comprehensive benefits package.
