About The Position
Under limited direction coordinates the plan of care for the patient and family through the continuum of care, in accordance with the CVRMC Clinical Resource Management Plan.
Requirements
- Must have either a current Arizona RN license or Clinical Social Work certificate in good standing, BSN or CSW preferred
- Must have at least 5 years of clinical experience.
- Recent case management experience in a healthcare setting preferred.
- Certification in Case Management by a recognized national Board preferred, and will be required within three years of hire.
- Must be able to interact effectively with co-workers, patients/family, physicians, and other hospital personnel.
- Must be able to perform effectively under physical and mental stress, maintaining concentration during that duration.
- Maintains current Case Management certification ( once acquired), maintains CPR.
Nice To Haves
- BSN or CSW preferred

- Certification in Case Management by a recognized national Board preferred
