Requirements

  • Education: Graduate of an accredited school of nursing or social work.
  • Experience: Minimum of three (3) years of hospital based nursing practice or social work experience.
  • Willing to pursue certification in Case Management.
  • Proficient in Computer Knowledge.
  • Licensure/Certification: Current RN license in the State of Oklahoma or graduate of a social work program.

Nice To Haves

  • Experience in hospital case management, discharge planning or social work a plus.

Responsibilities

  • The Case Manager/Discharge Planner is responsible for all activities associated with the discharge planning and coordination of care and service of individual patients and/or patient populations.
  • Promotes effective utilization of resources and assume a leadership role on the multidisciplinary team to achieve patient and physician satisfaction and optimal outcomes.
  • Must be able to work with all disciplines to meet patient needs from admission through discharge.
  • Will perform duties and responsibilities in a manner consisted with the mission and values of the Oklahoma Heart Hospital.
