About The Position
The Case Manager at Helen M. Simpson Rehabilitation Hospital is responsible for coordinating health care decisions to ensure effective treatment plans that enhance quality and outcomes. This role involves managing care across the continuum, promoting cost-effective care within designated time frames, and ensuring that hospital-based services meet patient-related utilization management criteria. The Case Manager also implements safe and appropriate discharge plans while assessing the psychosocial needs of patients and providing necessary interventions as part of the discharge planning process. Key functions include clinical interventions, care planning management, fiscal management, and payer/referral management.
