About The Position

Under general supervision, the HOPWA Housing Case Manager provides comprehensive, client-centered case management services to individuals and families living with HIV/AIDS. The primary focus of this position is to improve health outcomes, housing stability, and income attainment through coordinated supportive services. The Case Manager assists clients in obtaining and maintaining stable housing by conducting assessments, developing individualized service plans, coordinating rental assistance, and linking clients to medical, behavioral health, employment, and community resources. This role includes home visits, crisis intervention, documentation in HMIS and Avatar systems, and ensuring compliance with federal HOPWA program guidelines. The Case Manager serves as an advocate, liaison, and supportive presence while promoting client self-sufficiency and dignity.

Requirements

  • Trauma-informed care and harm reduction approaches
  • Strong organizational, communication, and documentation skills
  • Ability to work independently with minimal supervision
  • Cultural humility and experience working with diverse populations
  • Knowledge of HIV/AIDS-related issues and housing instability
  • Bachelor’s degree in social services or related field (Relevant experience may substitute for formal education)
  • Individuals with lived experience are strongly encouraged to apply
  • Valid state driver’s license and access to a personal vehicle
  • Ability to maintain strict confidentiality

Nice To Haves

  • 1–3 years of experience in homeless or residential services, Behavioral health or community mental health, and/or Housing case management
  • Experience with electronic health records and HMIS systems
  • Demonstrated passion for serving individuals living with HIV/AIDS
