About The Position

The Case Manager at Talbert House is responsible for overseeing the full client care cycle, which includes assessing, planning, implementing, monitoring, and evaluating actions to meet clients' health and human services needs. This role emphasizes client-centered care and requires coordination with offsite support to ensure services are safe, timely, effective, efficient, and equitable. The Case Manager will maintain a caseload, provide ongoing services, and assist clients in achieving wellness and autonomy through various supportive measures.

Requirements

• Bachelor's Degree in Behavioral Science or related field, or equivalent work experience.
• Ability to learn case management principles and healthcare management.
• Excellent organizational and time management skills.
• Effective verbal and written communication skills.
• Advanced problem-solving skills and ability to multi-task.
• Basic computer skills and proficiency in Microsoft applications.

Nice To Haves

• CDCA certification preferred.

Responsibilities

• Assess clients' needs and develop individualized service plans.
• Coordinate and provide timely and effective care to clients.
• Maintain regular contact with clients to monitor and assess their progress.
• Handle case assignments and draft service plans.
• Review case progress and determine case closure with growing independence.
• Deliver ongoing services including crisis intervention, advocacy, and medication monitoring.
• Assist clients with transportation, budgeting, housing, and referrals to community resources.
• Document all clinical activities accurately and timely to meet program and funder criteria.
• Participate in treatment team meetings and make recommendations for client status changes.
• Lead educational groups and facilitate group curriculum as needed.
