Requirements

  • The incumbent shall, at a minimum, be a Registered Nurse professional with 3-5 years clinical experience including med-surg and post-acute care settings or be a graduate of a CSWE approved master's degree program and hold applicable and current license/certification to practice.
  • ACSW certification is desirable.
  • Incumbent must hold applicable and current license/certification to practice.
  • Incumbent shall have demonstrated progressive responsibility in previous job roles, positive communication skills and basic computer word processing ability.
  • Knowledge of reimbursement benefits and requirements including but not limited to Medicare, Medicaid and managed care is required.

Responsibilities

  • Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
  • Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
  • Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Assists with discharge planning and processes.
  • Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
  • Mentors new staff, students, and interns.
  • Performs Medicaid screenings as indicated for NHP.
  • Assists patients with Safety Net applications when needed.
