About The Position

Responsibilities of the case manager include such practices as enrolling clients into select programs, assessing needs, implementing in-home evidence-based parenting/family strengthening curriculums, developing care plans as applicable, coordinating services, and providing follow-up and reassessment, as required. Data entry of client information into external and internal databases. and appropriate FSA client tracking documents. Participation in program outreach events, and other community networking as assigned.

Requirements

  • 1-2 years’ case management experience in a community-based setting preferred
  • Have excellent written and verbal communication skills.
  • Ability to communicate effectively via e-mails.
  • Ability to analyze and interpret information to case plan appropriate interventions.
  • B.A. in social work or related field
  • Valid CA Driver’s License and proof of Automobile Insurance
  • Working at a desk for extended periods of time with computer/keyboard work. Long periods of intense concentrated attention on details.
  • Ability to lift/carry at least 30 pounds.
  • Ability to perform tasks that require bending/stooping.
  • Ability to perform under high pressure/problem-solving situations.
  • Ability to cope with a fast-changing work environment.
  • Contribute to the organizational culture of the department with a positive attitude and professional demeanor.
  • Ability to multi-task and stay focused with many interruptions.
  • Ability to deal with many different personalities.

Nice To Haves

  • Experience with providing parenting classes/programs preferred
  • Bilingual/Spanish speaking preferred.

Responsibilities

  • Direct service treatment, including diagnostic (intakes), needs assessment, application of concrete services, and on-going care planning to individual, family, and group members.
  • Implementing in-home evidence-based parenting/family strengthening curriculums
  • Caseload management: information/referral advocacy.
  • Community outreach, presentations, workshops, and parent education training as necessary to facilitate individual case and program goals.
  • Generation and implementation of Individual Service Plans and other case planning activities.
  • Participation and presentation at case review/disposition, case consultation, and staff meetings.
  • Assisting in program start-up activities and subsequent (on-going) statistical and recording activities.
  • Provide services throughout Riverside County, as needed.
  • Compilation of statistics, report preparation and funding/contract source relations.
  • Punctual and Regular Attendance.
  • Other duties as assigned and necessary
