As a Case Manager in our Family Based Care team, you will work alongside children, young people, their carers, and families to create and carry out case plans that promote permanency and align with the best interests of each child or young person. You’ll also collaborate closely with the Department of Families, Fairness and Housing (DFFH) and other key services to ensure all support aligns with case plans and court directives. In this role, you'll be instrumental in promoting quality practice, and ensuring the safety, stability, and wellbeing of children and young people in care. Requirements

  • Relevant bachelor’s degree such as Social Work, Social Welfare, Psychology or similar
  • Strong knowledge of trauma-informed care and effective responses to pain-based behaviours.
  • Proven experience in case management within child protection, including developing and implementing case plans.
  • Skilled in managing complex caseloads and prioritising competing demands in high-pressure environments.
  • Culturally responsive practice when working with individuals, families, and diverse communities.
  • Excellent communication skills, with the ability to engage stakeholders and produce high-quality written documentation.
  • A current unrestricted driver’s licence.
  • Successful candidates will be required to clear probity checks including National Criminal History Record Check, VIC Working with Children Check and registration on the VIC Residential Carer Workers register (WCES)

Responsibilities

  • Case management for children in foster care and/or Targeted Care Packages (TCP), including living with parents, kinship care or independent living.
  • Building strong relationships with children, young people, and carers through regular home and outreach visits.
  • Including client voice in care planning and preparing them for leaving care.
  • Collaborating with care teams to meet health, development, and wellbeing needs.
  • Supporting and coaching foster carers to ensure a safe, secure environment for children & young people.
