About The Position

To provide a Case Management Service as part of the wider Disability Team that seeks to improve the health and social wellbeing of the service users. S/he will achieve this via an accessible, equitable, person-centred service. The successful candidate will endeavour to provide a co-ordinated, integrated approach to the delivery of services to each person with a disability.
