About The Position

This position is responsible for providing mobile case management services to individuals with serious mental illness and substance use disorder who are living in shelters, on the street, are without stable housing, or are at-risk of homelessness. The Case Manager-CPSS will draw on their own experience with mental illness to provide peer counseling and direct support, as well as model wellness and assist individuals in their recovery. As part of the team, will work in partnership and collaboration with social service and housing providers to ensure that individuals experiencing homelessness decrease the time between referral and being housed, make connections with other services as needed, and remain engaged.

Requirements

  • Knowledge of severe and persistent mental illness (SPMI) and the needs of people with SPMI.
  • Knowledge of local community resources and eligibility requirements. Resources needed include those pertaining to housing and healthcare for a variety of populations and needs.
  • Skills in a variety of treatment approaches including person-centered planning, harm reduction, and motivational interviewing.
  • Ability to develop and maintain receptive, open, and approachable lines of communication with clients and staff, and provide a friendly environment for visitors.
  • Strong written and verbal communication skills.
  • Presentation and meeting facilitation skills.
  • Ability to work as a proactive and positive member of a team to deliver quality services.
  • Ability to maintain confidentiality of organization and client data.
  • Ability to use sound judgment and maintain healthy professional boundaries.
  • Analytical and problem-solving skills.
  • Ability to meet deadlines and set priorities.
  • Proficiency with a variety of web-based software and databases, including Microsoft Office Suite and electronic medical records systems.
  • Qualified under Minnesota law as a Mental Health Rehabilitation Worker.
  • Successfully completed the Minnesota Department of Human Services (DHS) approved Certified Peer Specialist training and certification exam.
  • Must not be actively receiving services from Guild and must not have received services within the last two years.
  • A valid Class ‘D’ Driver’s License, reliable vehicle, satisfactory driving record and necessary insurance.
  • Current CPR/First Aid Certification is required.
  • Ability to pass a MN Department of Human Services background check.

Responsibilities

  • Connect with individuals referred to Guild via Suburban Metro Area Continuum of Care (SMAC) Coordinated Entry System.
  • Coordinate intake and perform an individualized needs assessment to develop a Housing Support Plan that addresses barriers to obtaining housing.
  • Connect individuals to suitable housing options like subsidized housing, Section 8, emergency housing vouchers (EHVs), single room occupancy (SROs), permanent supportive housing, affordable and market-rate housing, and other similar opportunities.
  • Assist individuals in accessing services and resources and coordinating housing related paperwork, with prospective landlords, ensure completion of Coordinated Entry Assessment for potential priority housing opportunities.
  • Provide crisis intervention to homeless individuals in the community or in a safe environment as needed.
  • Attend monthly training offered by the Department of Human Services (DHS) and other stakeholders or as needed.
  • Document all interactions and communications with individuals in an accurate and timely manner in the Homeless Management Information System (HMIS) and Guild’s electronic health records system (Credible).
