About The Position

Arms Acres is a licensed facility providing inpatient and outpatient treatment for those suffering from addiction to alcohol or drugs. The function of this position is to provide comprehensive clinical case management services that include assessments, individual, group and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems, including social service programs and managed care.

Requirements

• Required qualifications are CASAC-T, CASAC certification or LMSW, LMHC.

Responsibilities

• Provides/ensures comprehensive evaluation of patient's functioning as defined by facility policy and procedures.
• Coordinates the development and implementation of the multidisciplinary treatment plan; coordinates treatment plan review; performs these activities within established time frames.
• Documents patient progress and own interventions in case record.
• Provides individual, group and family treatment (psycho-education, life skills) as identified in patient treatment plans and requested by program director.
• Performs utilization review, provides progress reports and develops aftercare plans in conjunction with referring persons/agencies, EAPs and managed care/insurance entities.
• Attends and/or facilitates Family program when requested by supervisor.
• Responds to emergency situations in the facility by following appropriate procedures for ensuring patient safety during emergency procedures.
• Is actively involved in milieu management through attendance at community meetings, supervision of patients during activities and interventions with patients found in violation of program/facility rules.
• Maintains professional competence in discipline and treatment specialty areas through continuing education and supervision.
