The position involves providing intensive and professional case management services to youth, utilizing a strength-based and comprehensive model. The role is centered around linking children and families to essential resources and supports, monitoring the services provided, and actively advocating for the needs of children and families. The case manager will assist families in navigating through various systems of care, ensuring that they receive the necessary support and guidance throughout the process. The case manager will be responsible for developing and maintaining effective working relationships with families and community resources, including medical, mental health, and substance abuse professionals, child welfare personnel, juvenile justice personnel, lawyers, therapists, law enforcement staff, school officials, neighbors, and representatives from religious and youth organizations. This role requires conducting assessments to determine program eligibility, identifying problem areas, and recognizing areas of strength within the family unit. In addition, the case manager will maintain current knowledge of available formal services, resources, and supports in the local community, identifying needed services and linking youth to these services. The position also involves monitoring linked services to ensure quality of care and acting as a youth and family advocate, empowering them to advocate for themselves within the community. The case manager will assist youth and family members in navigating various systems, educating them to build their understanding and support permanency planning and outcomes. The role includes developing, monitoring, and reviewing service plans, maintaining appropriate levels of direct contact with youth and family members, and being available for crisis intervention as needed. The case manager will also create and manage discharge planning with a focus on safety, permanency, and well-being, including step-down placements, reunification, adoption, or transition plans. The position requires promoting a culture of professionalism through role modeling and respect, meeting with youth and family members at various locations, and utilizing their own vehicle for travel and transportation as authorized. Administrative functions for Care Coordination Services will also be part of the role, including preparing and submitting reports concerning case load status, budget requests, changes in service plans, and any unusual incidents. The case manager will ensure compliance with all contractual, regulatory, program, and accrediting body standards, and may be trained to become a certified Common Sense Parenting Instructor, conducting courses as assigned.
