About The Position
The Case Manager at Housing Connect plays a vital role in supporting the mission of connecting individuals and communities to quality affordable housing opportunities. This position is specifically focused on the Bud Bailey Apartment Community, where the Case Manager will assist in the development and implementation of supportive services tailored for refugee households. The Case Manager will collaborate closely with supportive services staff, property management, and community partners to identify the needs of residents and coordinate resources that promote self-sufficiency, housing retention, and a healthy community environment. In this role, the Case Manager will engage in tenant support and advocacy by developing and maintaining a comprehensive network of services that include education, training, mental health support, substance abuse resources, employment assistance, and economic development opportunities. The Case Manager will conduct home visits and outreach to residents, assess their employment status, and work collaboratively to set clear, measurable goals that facilitate their journey towards self-sufficiency. Building effective relationships with residents through regular communication is essential, as is providing education on housing stability issues. Additionally, the Case Manager will coordinate supportive services with community agencies, provide referrals, and follow up to ensure that residents receive adequate support. The role also involves program tracking and outcome measurements, where the Case Manager will maintain organized case files, complete necessary paperwork, and collect data to track resident progress. Overall, the Case Manager is expected to foster a sense of community among residents and support their engagement in educational and employment opportunities, ultimately contributing to their stability and success.
