About The Position

The Case Manager at Bronx Grove will maintain professional relationships with clients while ensuring compliance with HIPAA regulations. The role involves practicing Universal Precautions and adhering to security and privacy policies to protect health information. Responsibilities include reviewing documentation for tenant eligibility, conducting initial intakes and assessments, and preparing independent living plans in collaboration with tenants. The Case Manager will assist tenants in achieving their goals by identifying community resources and making referrals, as well as conducting group sessions to enhance independent living skills. Monitoring tenant progress, participating in case conferences, and providing necessary reports are also key aspects of the position. The Case Manager may act as a tenant advocate and assist with applications for benefits, ensuring ongoing support for tenants after achieving their goals.

Requirements

  • Bachelor’s degree (B.A.) and/or equivalent experience.
  • At least 1 year of experience working with the SPMI population.
  • Obtain Criminal History Checks (CHC) fingerprinting and maintain clearance.
  • Experience working with persons with HIV, mental illness, and/or substance abuse histories.
  • Flexible hours may be needed for shift coverage.
  • Experience in clinical supervision and housing issues.
  • Experience developing and facilitating groups.
  • Knowledge of supportive housing services operations.

Responsibilities

  • Maintain professional relationships with clients and compliance with HIPAA regulations.
  • Practice Universal Precautions/Standard Protocol & Procedures.
  • Comply with Federal, State, City, and CAMBA security and privacy policies.
  • Review documentation establishing tenants' eligibility for the program and make file copies.
  • Create and maintain tenant files.
  • Conduct initial intake and assessment of tenants and their families' needs.
  • Prepare initial and periodic revisions of independent living plans with tenants.
  • Assist tenants in attaining their goals by identifying and locating community resources.
  • Work with tenants to overcome barriers to their goals and advocate for themselves.
  • Conduct group sessions and/or workshops to assist tenants with independent living skills.
  • Monitor tenants' progress toward their goals and document via progress notes.
  • Participate in case conferences.
  • Follow-up with tenants and referral organizations regarding tenant progress.
  • Provide required information for reports.
  • Recommend case closures based on tenant progress or lack of participation.
  • Act as tenant liaison/advocate with outside organizations.
  • Schedule appointments for tenants with referral organizations.
  • Escort tenants to appointments as needed.
  • Assist tenants in completing applications for benefits.
  • Follow-up with tenants post-goal completion to ensure stability.
  • Evaluate living conditions of tenants through home visits.
  • Prepare marketing materials for the program.
  • Prescreen tenants for eligibility and schedule intake appointments.
  • Input tenant data into automated databases.
