About The Position
Hotel California by the Sea (HCBTS) is a dual-diagnosis drug and alcohol treatment center located in Huntington Beach, CA, with additional facilities in Bellevue, WA, and Cincinnati, OH. We are dedicated to treating our clients with the dignity, compassion, and respect they deserve while they are in treatment. We are currently seeking a dynamic, flexible, and passionate individual to join our outstanding team as a Case Manager. As a Case Manager, you will provide substance use disorder counseling and case management services to clients across the continuum of care. The services you provide will be individualized, evidence-based, culturally sensitive, and tailored to meet the specific needs of the individuals served. You will be responsible for completing all ASAM assessments, screenings, and discharge/transfer documentation in accordance with the current ASAM Treatment Criteria and Level of Care Guidelines. This includes conducting individual 1-on-1 case management sessions, ASAM Level of Care Assessments, group counseling, and group education sessions, as well as other case management services as needed. Your role will involve utilizing information obtained from various sources, including clients, medical records, physicians, psychiatrists, therapists, case managers, nurses, and family members, to evaluate and address the client's biopsychosocial needs across all six dimensions of the ASAM criteria. You will assess clients' strengths, needs, abilities, and preferences to develop a workable treatment and recovery plan. Ongoing coordination of the client's needs through the continuum of care will be essential, as will providing education to clients and their families about addiction and treatment options. You will also continuously review the client's psychosocial support system, living environment, and career/education status to identify barriers to treatment and recovery. In addition, you will support and participate in therapeutic treatment interventions, utilize de-escalation and crisis intervention techniques, and work collaboratively with the clinical team to achieve desired treatment outcomes. Active participation in clinical meetings with the multidisciplinary treatment team is expected, along with timely and professional documentation of all treatment and services rendered in the HCBTS EMR system (KIPU). You will also participate in continuous quality management and performance improvement processes, complete compliance reports, and assist in the clinical utilization review process as necessary.
