About The Position

The Case Manager Assistant collaborates and provides assistance and administrative support to the Case Managers and Social Workers. The Case Manager Assistant aids in the coordination of discharge planning, patient placement and referrals to post-acute care services. The Case Manager Assistant is able to function under the direction of a Case Manager/Social Worker with minimal supervision. Hours: Full-time, 8am-4:30pm

Requirements

  • HS diploma, CMA or C.N.A required
  • High School (Required)
  • BLS or CPR certification within 90 days of hire.
  • CMA-Certified Medical Assistant

Responsibilities

  • Answer and return phone calls
  • Assists in arranging any follow up appointments for post hospitalization
  • Check the case manager phone voicemail
  • Communicates with extended care facilities to retrieve pertinent patient information and to verify patient's advance directive, level of care, financial and other information critical to the timely discharge of the patient
  • Complete documentation in the medical record as appropriate
  • Coordinating and arranging post discharge transportation
  • Explains regulatory forms and obtains patient signatures
  • Helps to ensure timely referrals and timely patient discharges
  • Help to facilitate skilled nursing facility placements by faxing paperwork and making phone calls
  • Other work as delegated
  • Special projects as requested
  • Uploading and sending packets/updates to post-acute facilities, home health companies, etc.
  • Verifies information related to health insurance and benefits
  • Works directly with the nursing units to arrange DME for post discharge
