Job Summary: The Case Manager Assistant provides administrative, coordination, and documentation support to the Case Manager and interdisciplinary team. This role is non-clinical and does not perform clinical assessments or make service determinations, but supports resident services processes in accordance with New York State Department of Health regulations and Longview policies. The Case Manager Assistant also serves as a concierge to residents and their families, supporting communication, follow-up, and navigation of processes to promote resident-centered care, safety, and quality of life.

Requirements

  • High school diploma or equivalent
  • Previous experience in assisted living, long-term care, healthcare administration, or related support role
  • Strong organizational, documentation, and time-management skills
  • Basic computer proficiency, including experience with electronic health records and Microsoft Office or similar systems
  • Ability to maintain confidentiality and work with sensitive information
  • Ability to work effectively with residents, families, staff, students, and volunteers
  • Effective verbal and written communication skills
  • Satisfactory completion of post-offer, pre-employment physical required

Nice To Haves

  • Experience supporting admissions, regulatory documentation, or care coordination in an assisted living setting preferred
  • Familiarity with NYS DOH assisted living regulations and documentation processes preferred
  • Experience with assisted living management software (e.g., ALIS, Eldermark) preferred
  • Basic knowledge of resident rights

Responsibilities

  • Provide administrative and logistical support to the Case Manager and interdisciplinary team
  • Assist with admissions, transfers, discharges, and onboarding of new residents
  • Coordinate care plan meetings, resident reviews, and follow-up tasks
  • Prepare draft Individualized Service Plan documentation and assist with admission agreements
  • Track and maintain NYS DOH forms, waivers, and regulatory documentation
  • Enter and update resident information in the electronic health record (EHR)
  • Conduct non-clinical resident check-ins and follow-up with residents and families
  • Serve as a point of contact for routine questions and support resident integration
  • Communicate with staff, therapy providers, and external agencies as directed
  • Attend designated meetings and assist with special projects related to resident services
  • Maintain confidentiality of resident and operational information
