Reporting to the Manager of Case Management, the Case Manager Assistant works closely with the case manager to ensure the discharge process is smooth and has the responsibility for facilitating discharge services, discharge appointments, and DME needs. A successful case manager assistant is organized, communicates well, and can problem solve.
