Case managers are a critical part of the service delivery team, especially in assuring that the children's needs are being met and individualized for each minor. The Case Manager, Assistant Lead is responsible for assisting the Lead Case Manager in ensuring that weekly case management services are provided to the minors residing at the St. Michael's Home for Children program and that efforts are concentrated on safe and timely releases. The Case Manager, Assistant Lead will ensure that the children are being assessed and placed appropriately. Also, the Case Manager, Assistant Lead provides administrative oversight for the SMHC case management team's responsibilities, including ensuring productivity and quality assurance targets/goals. The Case Manager, Assistant Lead will also be responsible for the timely completion of daily, weekly, monthly, and quarterly reports.

Requirements

  • Bachelor’s degree in Social Work, Human Services, or a related field
  • Two (2) years progressive employment experience that demonstrates supervisory and case management experience which includes one (1) year of experience providing child welfare services to CPS, vulnerable, or at risk youth populations and one (1) year of experience in a leadership role.
  • Bilingual (English/Spanish)
  • Valid Texas Driver’s License
  • Strong verbal and writing skills
  • Ability to safely drive and transport clients on agency vehicles.
  • Ability to appropriately handle and maintain confidential information.
  • Able to prioritize work responsibilities, handle multiple tasks, and work in a fast-paced environment.
  • Demonstrate knowledge and understanding of professional boundaries with children in care, staff, and volunteers.
  • Capability and competence in providing individual, family, and group case management services.
  • Experience providing presentations and training.

Responsibilities

  • Ensures that weekly case management services are provided to the minors residing at the St. Michael's Home for Children program and that efforts are concentrated on safe and timely releases.
  • Ensure that children are being assessed and placed appropriately while in care and aftercare services.
  • Ensure case management services are being provided, including communication with family and identified sponsors and appropriate entities focused on children's release.
  • All case management services to children and families follow professional licensing standards, agency standards, accreditation standards, ORR requirements, and HIPAA.
  • Direct client service delivery includes but is not limited to the following case management duties: conducting assessments in person or on the phone, developing and implementing treatment plans, assessing risk and safety needs for possible sponsors regarding home studies or follow up services, and providing appropriate referrals.
  • Assist with data entry tasks associated with tracking measurable outcomes and management of staff performance.
  • Completes necessary daily, weekly, monthly, quarterly, and annual reports.
  • Manages and facilitates an individual caseload of 5 client cases and maintains documentation compliance.
  • Completes documentation of client progress within required time frames; maintains client records following regulatory and agency standards.
  • Trains new case managers and staff.
  • Assist with managing, developing, and updating documentation of services rendered.
  • Offers guidance and recommendations during weekly staffing of cases with treatment team members.
  • Contributes to the development of on going discharge planning for clients as their case develops for release from St. Michael's Home for Children Program.
  • Ensures the SMHC case management team meets all documentation requirements.
  • Attends Stakeholders meetings hosted by ORR and appointments as needed through ICE, private attorneys, and consulate officials to coordinate services.
  • Respond to and participate in emergency situations, including executing evacuation procedures and other safety protocols.
  • Adhere to all safety protocols and participate in regular drills; support evacuation and accountability of minors in emergencies.
  • Identify and escalate safety risks; contribute to behavior support plans and deescalation using trauma informed approaches.
  • Comply with incident reporting procedures and timelines.
  • Work a rotating schedule that may include evenings, weekends, holidays, and on call coverage; provide shift support to maintain required staffing ratios.
  • Flex between units/teams to meet program needs; cover peer caseload tasks when other case managers are off duty.
  • Engage in ongoing training and professional development related to child welfare, immigration, and case management best practices.
  • Complete and maintain required certifications (e.g., CPR/First Aid), program orientations, and annual refreshers (e.g., trauma informed care, emergency preparedness, confidentiality, child welfare best practices).
  • Perform other duties as required by supervisor.
