Assists project staff in facilitating linkage of clients from intake process to outpatient treatment

Requirements

  • High School or equivalent
  • Prior experience with ACT team, Mobile Crisis or similar community outreach service for persons with severe mental illness or experience with community-based approaches to substance abuse treatment necessary.

Nice To Haves

  • BA Preferred
  • Supervisory experience preferred.
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) preferred.
  • Issuing Agency: New York State

Responsibilities

  • Engages new clients in supportive group treatment
  • Participate as group therapist in ongoing treatment of clinic clients
  • Participates with team in delivering supportive psychiatric/substance abuse treatment to assigned clients
  • Maintains contacts with all appropriate agencies as necessary to sustain continued engagement and progress in treatment
  • Successfully tracks all assigned client's in the community and facilitates coordinated treatment for client's in contact with multiple agencies
  • Conducts community outreach visits with client's in residences, shelters, etc. to insure continued viability in outpatient treatment
  • Maintains supportive therapeutic contacts with otherwise resistant patients
  • Assists team in crisis intervention
  • Maintains accurate and up to date records
  • Documents all subject contacts, outreach visits and urine toxicology screens on a timely basis
