About The Position

The Case Manager position at ACCORD involves working with clients who have disabilities in Anoka, Hennepin, or Dakota County. The role focuses on enhancing the quality of case management services, maximizing client satisfaction, and promoting cost-effectiveness. The Case Manager is responsible for the entire care cycle of individuals, including assessing needs, evaluating treatment options, creating care plans, coordinating care, and monitoring progress to ensure that health and human services are met.

Requirements

  • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field, or a Bachelor's degree in any other field with at least one year of experience working with individuals with a mental illness or disability.
  • If holding a Social Work degree, must be currently licensed as a Social Worker as required by the Minnesota Board of Social Work.
  • Good written and oral communication skills.
  • Ability to work independently and in crisis situations.
  • Comfortable with high-volume paperwork.
  • Attention to detail and process-oriented.
  • Able to pass a DHS background check, possess a valid driver's license, and have a clean driving record.
  • Reliable transportation.

Responsibilities

  • Developing a community support plan (goals) for each individual.
  • Assisting individuals to access services and develop service agreements and documentation through case notes.
  • Informing individuals or legal guardians of service options that fit their needs.
  • Meeting one-on-one with each individual at least twice a year to evaluate progress and adjust needs as necessary.
  • Identifying potential providers to guide individuals towards their goals.
  • Evaluating and monitoring the services identified in the plan.
  • Ensuring compliance with case management rule 185 and MN Health Care Programs regarding waiver services.
