About The Position
Join our Snohomish County Human Services Family! OPEN ONLY TO HUMAN SERVICES EMPLOYEES. We have 1 (one) hybrid/remote Case Manager opportunity in our Aging and Disability Services Division. Snohomish County Human Services is hiring qualified individuals to coordinate long term services and supports for elderly and disabled individuals to prevent institutionalization and help them maintain living independently in the community. Our goal is to assist individuals to access the necessary services and advocate for them as needed. You have a desire to provide in-home care services to elderly and disabled adults, so they may live independently in the community. You will help individuals develop a service plan to increase their quality of life and prevent institutionalization. The Case Manager maintains ongoing contact with clients and will assess their needs annually and as needed to develop a plan to live independently.
