About The Position

The Adult Mental Health Services Case Manager at Gulf Bend Center is responsible for providing psychosocial rehabilitation services, skills training, and routine case management to adults with severe and persistent mental illness. The role involves completing assessments, treatment planning, and coordinating community services while acting as a liaison between clients and service providers. The position requires travel within the 7 counties and is performed under the general supervision of the Adult Mental Health Program Manager.

Requirements

  • Bachelor's degree from an accredited college or university in social, behavioral, or human services, or registered nurse.
  • Demonstrated ability to interview and assess clients using appropriate tools.
  • Ability to read and understand assessments and evaluations.
  • Ability to identify community resources and coordinate services.
  • Effective communication skills, both verbal and written.
  • Ability to work independently under general supervision.

Nice To Haves

  • At least six months' experience in providing mental health services to adults, children, and adolescents.

Responsibilities

  • Participates in the screening of new service requests within the clinic.
  • Accepts newly assigned cases after diagnostic intake by a licensed provider.
  • Completes case management assessments for all clients entering caseload.
  • Develops treatment/service plans with clients.
  • Provides ongoing case management functions according to service plans.
  • Conducts face-to-face assessments with clients quarterly.
  • Documents all service contacts in a timely manner.
  • Participates in integrated care planning and service coordination activities.
  • Maintains communication with psychiatrist and nursing regarding medication compliance.
  • Meets the required 80 hours of face-to-face contact a month.
  • Manages time effectively to complete job requirements.
  • Completes assessments two weeks before expiration.
  • Meets with all assigned engagement customers per contract measure.
  • Attends and participates in all team meetings.
  • Completes non-clinical needs assessments within 7 days of discharge from hospital.
  • Meets with new intakes within 7 days to provide case management services.
  • Maintains compliance with required education courses and training.
  • Performs other duties as assigned by the supervisor.
  • Maintains regular and reliable attendance.
  • Complies with telework/remote agreement.
  • Uses a trauma-informed approach when providing services.
