Case management is incorporated with the community support program and is designed to assist people with a serious and persistent mental illness to become independent and functioning members of the community. Case managers facilitate and broker services to assist clients in the promotion of independent living and teach and train clients to become more independent. Under the supervision of the Adult Services Program Manager, the case manager engages adult clients with Serious and Persistent Mental Illness (SPMI). In this position, the case manager will gather information, assess the clients need based on their individual functioning in the community, complete necessary referrals, collaborate with other professionals, and support clients through various transitions from other entities such as jails, hospitals, or residential settings, to assist in stabilizing their mental health. The case manager will have monthly contact with clients and maintain concurrent documentation to abide by state and CCBHC requirements.
