About The Position

The Case Manager for Adults at MiSide provides essential case management services to adult clients receiving outpatient mental health services. This role focuses on assisting clients with individualized care coordination and developing integrated treatment plans in collaboration with clients and other service providers. The position emphasizes linking and coordinating services, referrals, reassessments, follow-ups, and advocacy to ensure comprehensive support for clients with psychiatric disorders.

Requirements

• Bachelor's Degree in Social Work.
• State of Michigan professional license (LLBSW, LBSW, LLMSW, LMSW).
• One year of case management experience working with adults with psychiatric disorders.

Nice To Haves

• Bilingual proficiency in Spanish.

Responsibilities

• Provide case management services to an assigned caseload of adult clients.
• Assist clients with case management and care coordination needs that are goal-oriented and individualized.
• Develop integrated treatment plans in collaboration with clients and other service providers.
• Link and coordinate services, referrals, and reassessments for clients.
• Conduct follow-ups and advocacy to monitor services provided to clients.
