The case manager position is designed to assist person served in identifying and acquiring both natural and paid supports which allow them to live as independently as possible in the community. The case manager should assist the individual in a person centered approach, in a way which supports the person in making his/her own choices and decisions about these supports. The case manager should function as an educator and a resource as well as service coordinator. Life decisions should be made by the individual and/or guardian. The case manager will focus on coordinating services for vulnerable individuals and assisting them to navigate the complex service delivery system. The position will provide a systematic process of on-going assessment, planning, referral, service coordination, monitoring, consultation and advocacy assistance, through which multiple service needs of individuals are addressed. This position will be responsible for directly coordinating a caseload, and facilitate/coordinate other services or supports, which will positively influence individuals' lives. The case manager will have contact with a wide range of organizations, families, and other community members. The case manager will act in a professional manner at all times, and understand that they represent the interests of the individual they support.
