About The Position

BronxWorks Homebase assists more than 3600 households with housing retention and stabilization services in the South Bronx. We achieve this by providing services to include case management, public benefits advocacy, referrals to available resources, mediation, and financial literacy workshops. BronxWorks is seeking a highly motivated Homeless Prevention Case Manager to join our Homebase team. As a case manager, you will play a critical role in helping individuals and families at risk of homelessness stay stably housed or relocate to stable housing. Your responsibilities will include screening and assessing client needs and developing strategic interventions to stabilize clients' housing. You will be expected to engage clients, assess for program and/or subsidy eligibility, develop service plans, conduct Housing Court advocacy, document progress, complete virtual/in person unit inspections, and follow up with external parties. You will connect clients with resources like rental arrears assistance, public benefits and housing vouchers. You will manage a caseload of approximately 40-45 clients in various stages of a housing crisis. The ideal candidate will thrive in a fast-paced environment, display an ability to multi-task, be flexible to programmatic change and adapt accordingly, be extremely organized, and be adept at working with multiple software platforms. We are looking for a desire to work with an at-risk population, strong assessment and analytical skills, and excellent verbal and written communication abilities. Requirements

  • Bachelor’s degree in social work or another related field of study.
  • Bilingual language skills are preferred.
  • Strong oral and written communication, time management, and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard software.

Responsibilities

  • Provide homeless prevention case management services to households in the South Bronx.
  • Perform initial intake assessments for clients and their families, as well as periodic reassessments including developing and updating comprehensive service plans.
  • Review all documentation to determine clients’ eligibility for program, and maintain both physical and digital folders.
  • Assist clients in completing all BronxWorks Homebase Program intake applications and forms.
  • Refer clients to appropriate services not limited to within BronxWorks.
  • Follow up with clients and referral agencies to track engagement and progress.
  • Conduct home visits and inspections to assess clients’ living conditions (in person and/or virtually).
  • Enter client information and progress updates into designated database systems.
  • Support clients in overcoming barriers to achieving their goals and in building self-advocacy skills toward self-sufficiency.
  • Conduct outreach and community engagement efforts to recruit new clients.
  • Recommend and implement strategies to encourage client participation throughout all processes.
  • Monitor and document clients’ progress toward their goals via regularly scheduled calls and/or face-to-face home/office visits.
  • Recommend case closure when clients have either met their goals and maintained stability, disengaged from services, or are found ineligible.
  • Meet assigned daily/weekly/monthly performance targets in alignment with funder/agency requirements.
  • Prepare and submit applications for subsidies, transfers, benefit applications, referrals, and financial assistance as needed.
  • Provide accurate data and documentation for required program reports on a weekly/monthly/quarterly and annual basis.
  • Accompany client to medical and/or entitlement appointments, if requested.
  • Work with landlords/management companies to assist clients in resolving their housing crisis.
  • Perform other duties as assigned by the manager.
