About The Position

The Case Manager 1 is responsible for delivering case management services to the Women & Children Substance Use Treatment Facility. Duties include intake assessments, development and implementation of individual care plans, liaison with treatment providers and community resources, facilitation and documentation of client and group activities as well as daily psychoeducation groups in accordance with Women’s and Children’s curriculum and DMHAS core topics. The Case Manager 1 is a member of the care team providing care and comprehensive substance use treatment services to women who have young children or are expecting a child, at our residential Women and Children Substance Abuse Treatment facilities.

Requirements

• Some post secondary education
• 2 years relevant experience
• Valid Connecticut Driver’s License
• Regular and predictable attendance is required
• If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
