Selected candidates will supervise, evaluate, train and coach case management staff who are providing professional case management services. The supervisor will ensure staff are addressing client social determinants of health, closing HEDIS gaps, and providing care coordination with internal and external medical providers.

  • Supervise/evaluate team of case management providers to maintain quality case management service.
  • Monitor/report complaint issues according to grant/contract requirements relating to case management services.
  • Provide professional case management services on an as needed basis.
  • Audit staff charts to ensure compliance with appropriate licensing, CARF, and MCO standards.
  • Participate in scheduled audits with payer sources.
  • Maintain client confidentiality according to Center policies.
  • Remain current in knowledge of community resources and how to access those resources.
  • Keep supervisors apprised of all critical or emergency situations.
  • Responds to emergency/crisis or walk-in situations calmly, rationally and in accordance with center policy and procedures.
  • Participate in inservice training and orientation and provide consultation and education services to the community as requested by supervisor/administration.
  • Be involved in the admission, hospital stay and discharge of individuals on caseload who are admitted to an impatient psychiatric facility.
  • Be available to consumers 24 hours a day for crisis assistance.
  • Attend and participate in regularly scheduled staff meetings and inservices and individual program planning staffings as needed.
  • All other duties as assigned.
  • Bachelor's degree in mental health related field required.
  • Master's degree in mental health related field preferred.
  • Must possess or obtain highest level of licensure appropriate to degree.
  • Valid driver's license.
  • Must possess or obtain highest level of certification appropriate to degree.
  • Experience in case management, mental health, substance abuse or intellectual disabilities preferred.
  • Verbal/written communication skills.
  • Ability to maintain composure in difficult situations.
  • Ability to safely operate motor vehicle.
