About The Position

The DeKalb County Health Department is seeking a full-time (37.5 hours per week) Case Management Specialist to support maternal child health programs in the Community Health and Prevention Division. This position reports directly to the Women, Infants and Children (WIC)/Better Birth Outcomes (BBO) Coordinator. Ideal candidates have excellent communication skills, are detail oriented, organized and possess an interest in improving the health in our community.

Requirements

  • Bachelor's degree in Nutrition, Dietetics, Public Health, Health Science or a closely related field. OR Current licensure as a Registered Nurse (RN).
  • Minimum of three (3) years' experience in public health, social services or community nutrition preferred.
  • Excellent written and oral communication and customer service skills.
  • Ability to problem solve, be flexible, collaborate and maintain effective working relationships.
  • May require some early evening hours and off-site clinics.

Nice To Haves

  • Bilingual in English and Spanish preferred.

Responsibilities

  • Independently manages a caseload of high-risk pregnant and postpartum individuals and their infants.
  • Completes comprehensive prenatal, postpartum and infant assessments, including social determinants of health, physical assessments and social history assessments.
  • Recognizes and interprets client needs, including medical, mental health, nutritional and social needs and coordinates or facilitates timely referrals and follow-up services.
  • Provides client-centered education, including but not limited to, safe sleep practices, reproductive health and family planning, WIC nutrition assessment and education and health literacy.
  • Provides services in clinic, field and community-based settings, including both office and home visits.
  • Maintains accurate and timely documentation and manages follow-up activities based on client needs.
  • Implements BBO-C initiatives, including off-site clinics, community resource navigation and development of community partnerships to support referrals and continuity of care.
  • Participates in outreach and WIC initiatives, including late clinics, off-site and pop-up events.
  • Maintains positive and professional relationships with clients, community partners and staff.
  • Performs other duties as assigned.
