About The Position

The Case Management Registered Nurse (RN) plays a critical role in coordinating and managing patient care to ensure optimal health outcomes. This position involves assessing patient needs, developing individualized care plans, and collaborating with multidisciplinary teams to facilitate effective treatment and resource utilization. The RN will serve as a liaison between patients, families, healthcare providers, and community resources to promote continuity of care and support patient advocacy. By monitoring patient progress and adjusting care plans as necessary, the Case Management RN helps reduce hospital readmissions and improve overall patient satisfaction. This full-time role requires a compassionate, detail-oriented professional dedicated to enhancing the quality and efficiency of healthcare delivery across diverse patient populations in the United States.

Requirements

  • Current and valid Registered Nurse (RN) license in the United States.
  • Bachelor of Science in Nursing (BSN) or equivalent nursing degree.
  • Minimum of two years of clinical nursing experience, preferably in case management or a related healthcare setting.
  • Strong knowledge of healthcare systems, insurance processes, and patient care coordination.
  • Excellent communication, organizational, and critical thinking skills.

Nice To Haves

  • Certification in Case Management (CCM) or Accredited Case Manager (ACM) credential.
  • Experience working with diverse patient populations and chronic disease management.
  • Familiarity with electronic health records (EHR) and case management software.
  • Advanced training in patient advocacy, motivational interviewing, or health coaching.
  • Master s degree in Nursing, Healthcare Administration, or a related field.

Responsibilities

  • Conduct comprehensive assessments of patients medical, psychosocial, and environmental needs to develop personalized care plans.
  • Coordinate with physicians, specialists, social workers, and other healthcare professionals to implement and monitor care plans effectively.
  • Facilitate communication among patients, families, and healthcare teams to ensure understanding and adherence to treatment protocols.
  • Identify and connect patients with appropriate community resources, support services, and follow-up care to promote recovery and wellness.
  • Monitor patient outcomes and adjust care strategies as needed to optimize health results and prevent unnecessary hospitalizations.
  • Maintain accurate and timely documentation of case management activities in compliance with regulatory standards and organizational policies.
  • Educate patients and families about disease management, medication adherence, and health maintenance to empower informed decision-making.
