About The Position

The Care Manager Representative plays a pivotal role in assessing members' needs, advocating for essential services, and ensuring progress through continuous support and follow-up. By working closely with members and community organizations, the Care Manager Representative helps bridge the gap between individuals and the resources that empower them to grow and succeed. In this 100% remote role, the Care Manager Representative will contact members telephonically and via text to help schedule primary care and/or specialist appointments. The Care Manager Representative will also provide health education, conduct health risk assessments, and provide social determinants of health-related resources. Requirements

  • Experience in case management, SDoH, and/or healthcare advocacy.
  • Call center experience or remote customer service experience.
  • Strong communication and interpersonal skills with the ability to build trust and rapport with diverse community members.
  • Excellent organizational and time management abilities to handle multiple cases effectively.
  • Ability to collect, analyze, and report on data.
  • Knowledge of community-based services and social programs.
  • Demonstrate ability to manage multiple priorities in a fast-paced environment.
  • Conflict resolution skills and people management are required.
  • Ability to build trusting relationships
  • Commitment to the values of LOVE, LEARN, GROW, and a passion for supporting vulnerable populations.

Nice To Haves

  • Degree in Human Services, Social Work or related field.

Responsibilities

  • Member Needs Assessment: Conduct intake interviews to determine member needs, ensuring each individual receives personalized support tailored to their unique circumstances.
  • Advocacy: Serve as an advocate for members by connecting them with essential services and resources, helping them navigate social, medical, and community-based programs.
  • Progress Tracking: Monitor and document each member’s progress, ensuring consistent follow-up to address evolving needs and challenges.
  • Data Collection & Reporting: Collect, analyze, and summarize treatment results and compile statistics, providing accurate and timely reports for Wider Circle leadership to track program effectiveness and outcomes.
  • Collaboration: Work closely with community partners, healthcare providers, and social service organizations to ensure members receive comprehensive care, in line with Wider Circle’s commitment to LOVE and support for its community members.
  • Member Empowerment: Educate members on available resources, guiding them through processes that foster LEARNING and self-empowerment.
