Requirements

• High School Diploma/GED
• Must be able to read, write, speak and understand English.
• Computer operation, organizational skills, ability to communicate well and prioritize, prior health care experience

Responsibilities

• The Case Management Care Coordinator assists in the clerical functions of the discharge planning process
• the coordination, collection and recording of Case Management information in compliance with Medicare, Medicaid, and third party payers
• ensuring discharge needs are met and that the patient and their families understand the availability of resources for their medical and social support and education.
